Reservation Preference for Pavilion Usage
|
To reserve the pavilion you must complete and return a Rules
for Practice Pavilion Use Form. Click here for a copy of the form.
Anyone interested in reserving time in the Practice Pavilion can do so by
contacting Tom Follis by phone 661-4163 or email at jfollis1@wi.rr.com.
Every player using the pavilion under your supervision must have a
signed Informed Consent / General Release Form on file with BLL. Click
this Link Informed Consent /General Release Form for a copy of the form.
Times will not be reserved until these forms are on file with BLL.
The Pavilion may be reserved by families in one hour time slots for the upcoming
month. Reservations will be taken one week prior to the end of the month for the
following month. Preference will be given in order to BLL teams, BLL members and
then non-members may reserve any remaining available times during preferred hours
(Monday-Friday from 5:00 p.m.-9:00 p.m. and Saturday-Sunday from 10:00 a.m.-8:00 p.
m.). Individual BLL members/families who reserve the pavilion do not do so exclusively
and may be grouped with other BLL members/families up to a total of 5 players. All
players must have a signed Informed Consent/General Release Form on file
prior to using the pavilion.
Reservations for non-members during preferred hours will be considered confirmed two
weeks prior to the date, up until two weeks prior to their date they may be bumped by
BLL Teams or BLL members. Non-members can reserve times on a monthly basis and
consider them confirmed during non-preferred hours (Monday-Friday prior to 5:00 p.m.
or from 9:00 p.m.-10:00 p.m. and Saturday-Sunday from 8:00 a.m.-10:00 a.m. or 8:00 p.
m.-10:00 p.m.).
Cost to use the pavilion for non-members and non-member teams is $25/hour paid in
advance. Prior to using the pavilion each non-member individual must pay a
one-time $25 fee and turn in a completed Informed Consent/General Release
and Authorization form. Use of the facility by players without a completed Informed
Consent/General Release form or who have not paid their $25 fee will result in loss of
the use of the facility by that player and the supervising adult and their team. It is the
supervising Coach’s responsibility to make sure all their players have met this
requirement, and to forward a list of players on their team that will be using the pavilion
to BLL.
Prior to using the pavilion you must have confirmed you have reserved a time slot. Use
of the pavilion without a reserved time slot will result in the loss of the use of the facility.
Any adult who will be in charge of the cage must have a signed Rental/Rules
Form on file with BLL prior to reserving the pavilion.
Please complete the
forms below as needed
prior to reserving the
pavilion.
Reservations will be made
based on the time of the
request. I will contact you to
confirm your reservation.
Please do not assume you
have a time reserved until it is
confirmed. I will try to update
the schedule daily, but can
not guarantee that it will be
done that frequently.
With the demand for cage
time increasing as teams start
to practice for the upcoming
season we will be grouping
up to 5 players in the cages
during each time slot for
individual reservations.
